All About My Dancer

“Get to know you forms” are an inclusive practice that many child care centers and schools implement. As the only inclusive dance studio in our area, we adopted this practice so that we can prepare for the varying needs of all of our dancers.

  • Optional form (however everyone is encouraged to complete it).

Credit Card Payment Authorization Form

There are 2 ways to add your credit card number to your account.

1. By completing a Credit Card Authorization and giving it to our lobby staff.

2. Logging into parent portal, typing in your credit card information, and saving it.

Recital Opt Out Form

  • All of Goins Dancing’s Dance Classes (including Tumble classes) perform in the Spring Dance Recital each year.

  • We understand that some dancers or their parents prefer to not participate in the recital.

  • When this happens, we do not charge a costume fee to the account.

  • To assure that we have accurate records of which dancers are not participating in the recital and that we do not charge a costume fee, please complete the following form.
